About Test:

SARS-CoV-2, the causative agent of Corona Virus Disease 2019, is an enveloped, single-stranded RNA Betacoronavirus. The Corona virus genomes encode 4 main structural proteins : spike (S), envelope (E), membrane (M) &nucleocapsid (N). The  S protein is a very large transmembrane protein that assembles into trimers to form the distinctive surface spikes of corona viruses which has high immunogenicity and has two parts s1 & s2. 66% immunogenicity is by S1. The SARS-CoV-2 Antibody to Spike Protein test detects Spike protein Receptor Binding Domain (S1). So it is a good target for antibody testing.

Spike protein is the focus of vaccines and convalescent plasma therapy, so levels of these antibodies correlate to immunity status.  This test should be done at least 15 days after the 2nd dose of vaccination.

What is the difference between this test and the older antibody test?

This test measures antibodies against the spike(S) protein of the virus, while the older total antibody test measures antibodies against the Nucleocapsid(N) Antigen on the virus. The total antibody test measured whether or not you had been exposed to the SARS-CoV-2 Virus and not whether you have protective or neutralizing antibodies. The newer test directly correlates with the level of neutralizing or protective antibodies that you have in your blood. There have been cases where the total antibody test is negative while the neutralizing antibody test shows a high response.

Neutralizing antibodies primarily target S protein in coronaviruses, in particular the S1 subunit and the RBD contained within the S1 subunit, preventing viral entry into the host cell.  Most vaccines target this subunit and induce an immune response against this subunit. The antibody test to Spike protein therefore measures this immune response and can be done post vaccination.

So which Antibody test should one do? Total Antibody or Antibody to spike protein?

If you want to simply check exposure status in the last few months, then a total antibody test or a spike protein test will work. If you have had COVID-19,been vaccinated, and want to check your neutralizing antibody levels, then the Spike protein test would be more appropriate.
