About Test:

When lung imaging shows some problem, BAL fluid testing may be ordered by your treating physician to find out the exact nature of the disease. BAL or bronchoalveolar washing is done to collect samples from the lungs. BAL solution contains cells and other substances like bacteria which are tested in a laboratory. It helps in diagnosing different lung diseases like lung infections, TB, interstitial lung diseases, connective tissue diseases, fat embolism, lipoid pneumonia, occupational lung disease, pulmonary alveolar proteinosis, sarcoidosis, and cancers (bronchus, lung, or metastasis).

As the name suggests, cell counts in BAL fluid is reported. The report includes presence or absence of red blood cells (RBCs), total WBC count, and differential count – the percentage of neutrophils, eosinophils, and lymphocytes in BAL fluid. Macrophages, mesothelial cells, and any other type of abnormal or malignant cells are also reported.

Depending upon the underlying pathology, the final report varies from being non-infectious to infections to lung cancer or metastasis. Any test results need to be interpreted after complete clinical correlation to reach a final diagnosis.

Medical condition:

BAL fluid testing helps us to know the status of smaller bronchi and lung alveoli. It is done to see for pathological nature of the lung disease when the patient is having symptoms like persistent cough, fever, breathing difficulty or hemoptysis (blood in coughing). It can also be done in patients having any immune system disorder like HIV-AIDS or diabetes. Low immunity poses a higher risk for certain lung infections. Once cause for the symptoms is known, appropriate treatment can be started.

Test preparation:

As per the clinician’s instructions, a patient needs to fast (not eat or drink anything) for few hours before undergoing this BAL procedure.

The sample is collected by a clinician usually a pulmonologist (lung specialist) in a hospital or on an OPD basis under sterile aseptic conditions while doing bronchoscopy. A saline solution is put through a thin, tube-like lighted structure called a bronchoscope which washes the airways and is sucked up into the bronchoscope. This saline solution is called BAL fluid. The sample is collected in EDTA or plain sterile container and sent to the laboratory for testing.

1) What is bronchoscopy?
 
It is a procedure that allows the doctor to see the respiratory tract and lungs with the help of a bronchoscope. Bronchoscopy is usually done by a pulmonologist. Tube is put through mouth or nose down the throat into airways, bronchi and finally lungs. Any disease along the path of bronchoscope is detected and when possible, is simultaneously treated like removing a blockage or any growth/tumor. BAL can also be done while doing bronchoscopy. Abnormal bronchoscopy indicates the presence of lung disease.
 
 
2) What are the indications of doing BAL?
 
Indications for BAL are:
a) Long-standing, non-resolving pneumonia.
b) Diffuse lung infiltrates in the lung alveoli or surrounding interstitium/lung parenchyma.
c) Any suspicion of alveolar hemorrhage.
d) In ventilator-associated pneumonia for doing culture examination.
e) Any lung infiltrates in immunocompromised/weak immune system patients.
f) At times, for research purposes.
 
 
3) What all infections can be diagnosed by BAL?
 
BAL helps in diagnosing lung infections like bacterial pneumonia, TB, Nocardia, pneumocystis Jirovecii, Legionella, fungal (Histoplasma, Cryptococcus, aspergillus), parasitic (hydatid cyst), and viral (cytomegalovirus). 
 
 
4) How does normal BAL fluid look like?
 
The main constituents of BAL are secretions from lung air spaces called as alveoli. The usual cell count is approximately 100cells/µL with the majority being macrophages (>80%), followed by lymphocytes (<15%) and only a small percentage is neutrophils (<3%) and eosinophils (<1-2%).
 
 
5) What is done in the fluid microscopic examination?
 
Microscopic examination is done to study the cells in BAL fluid. Total and differential WBC count is given. Cell count is used for diagnosing infections. Increased neutrophils suggest active infection within the alveoli. However, neutrophilia may also be seen in ARDS, connective tissue disorder, Wegener’s granulomatosis, pneumoconiosis, or pulmonary fibrosis. Eosinophils may be higher in infections (parasites, fungal), bronchial asthma, interstitial pneumonias, and idiopathic hyper-eosinophilic syndrome. Lymphocytic predominance points towards TB, HIV infection, malignancy, hypersensitive pneumonitis, occupational disease, and sarcoidosis. High levels of eosinophils and lymphocytes can be drug-induced or in connective tissue disorders. The presence or absence of RBCs is reported. In case of alveolar hemorrhage, each subsequent BAL wash will continue to have RBCs. High RBC counts is seen in recent hemorrhage. Alveolar hemorrhage beyond 48 hours shows hemosiderin-laden macrophages. Other cellular components like mesothelial cells, macrophages, atypical and malignant cells, if present, are mentioned. Amorphous-looking material on slide microscopy with acellular debris favors the diagnosis of pulmonary alveolar proteinosis. A reduced number of alveolar macrophages is seen in sarcoidosis.
 
 
6. Do I need to add any other tests?
 
When microscopic examination suggests the presence of abnormal cells, you need to carry out cytology examination of the fluid and a special type of staining called as PAP stain to rule out cancers like primary lung cancer, lymphoma, lymphangitic carcinomatosis or metastasis (spread from other organs). For any infective cause, microbiological tests like Gram Stain, acid-fast (ZN) stain, and fluid culture examination are required to see for the causative organism. TB culture or Gene Expert assay may be needed for a definitive diagnosis of TB. Based upon culture reports, antibiotic treatment can be started.
 
 
7. What are the advantages of BAL?
 
BAL helps us to study cellular as wells as non-cellular constituents of the lower respiratory tract and lung alveoli. It acts as a window to evaluate lung parenchyma and any alteration in BAL fluid reflects pathological changes in the lungs. It gives an idea regarding the nature of lung diseases.
 
 
8. Is there any risk in doing the BAL test?
 
There is very little risk involved while undergoing BAL procedure like sore throat for a few days, cough, transient fever, and very rarely bleeding in airways, infection, bronchospasm, or lung collapse in some parts.
