About Test:

CD16+CD56 are a marker for natural killer (NK) cells. NK cells are a type of lymphocyte that develop in the thymus gland and play an important role in innate immunity. They possess few properties and markers of T-cells but are not completely called T-cells. NK cells are involved in regulating the body’s response during viral infection and tumour surveillance. They act as the first line of defence and primitive response as these cells can work without requiring any initial antigen presentation. NK cells secrete cytokines like interferon-gamma and alpha which act upon cells of the immune system (macrophages and dendritic cells) thereby enhancing the body’s immune response (adaptive immunity). This test is about measuring circulating NK cells in the body and sees their levels in various congenital and acquired diseases.

 

This test is done by Flowcytometry analysis. In this test, first, the lymphocytes are separated from other white blood cells using CD45 gating. Then T lymphocytes expressing CD3, CD4, CD8 and B lymphocytes expressing CD19 are separated. Subsequently, the absolute value and percentage of NK cells in a given sample is reported. Biological reference range for CD16+CD56 (NK-CELLS) % is 02-27% and absolute count is 27-693 Cells/uL.

 

All reported values need clinical correlation. High NK cell levels in women of the childbearing age group are at a greater risk of pregnancy loss. Low values or deficiency increases the susceptibility to getting a viral infection with the herpes virus. Reducing levels of NK cells in HIV1 infection correlates with the disease progression.

Medical condition:

Various indications of doing NK cell testing is as follows:

1. Women with fertility problems, bad obstetric history and repeated abortions.

2. Assessment of patients suspected to have primary immune deficiencies due to NK cell defects.

3. Predicting prognosis and starting antibiotic treatment against opportunistic infections in patients with T cell immunodeficiency.

4. Assessing patients undergoing solid-organ transplantation and looking for immune reconstitution after bone marrow or hematopoietic cell transplantation.

5. Identification of chronic viral infections like Hepatitis C and HIV (human immunodeficiency virus).

6. Evaluating effects of Epstein-Barr virus (EBV) and human T-lymphotropic virus type 1 (HTLV-1) infection.

7. Lymphoproliferative disorders (LPDs) expressing NK cells include chronic NK-cell lymphocytosis, aggressive NK-cell leukemia, large granular lymphocytic leukemia, blastic NK-cell lymphoma, extranodal NK/T-cell lymphoma and hepatosplenic T cell lymphoma ( mucosal or cutaneous gamma/delta lymphoma). Few of these cancers are EBV positive.

8. Evaluation of NK cells in solid tumours like retinoblastoma, medulloblastoma, astrocytoma and neuroblastoma.

1. What is lymphoma?

Lymphomas are cancer that arises in lymphocytes. Lymphocytes are WBCs present in lymph nodes, thymus, spleen, bone marrow and specific lymphoid pockets all over the body. They are immune system cells that have varied functions right from fighting and preventing infections to the healing of infections and in autoimmunity. Lymphomas are of 2 types, viz. Hodgkin and Non-Hodgkin. In lymphomas, lymphocytes change, grow in an uncontrolled and uninhibited fashion, each category affects a different type of lymphoid cell and they show variable responses to therapy.

 

2. What is leukemia?

Leukemias are cancer of blood-forming cells inside the bone marrow. Blood cells are constituents of blood which includes red blood cells, white blood cells and platelets. Their precursors are present in the marrow in an immature form which undergoes maturation to get released into the blood.

 

3. What is flow cytometry?

Flow cytometry is a highly specialized laboratory test done on a cytometer with the help of laser beam and fluorescent dyes or tagged substances (antibodies) to identify the types of cells in the blood, bone marrow or lymph node samples. It can also identify particular components within the cell and count a specific number of cells. Its most common use in medicine is immunophenotyping and diagnosis of leukemia and lymphomas.

 

4. What does CD stand for?

CD stands for cluster of differentiation which is also known as cluster of designation or classification determinant. The pattern of surface CD antigen expression indicates the degree of maturity. It is a protocol utilized to identify and investigate molecules present on cell surfaces. This subsequently helps in providing targets for immunophenotyping of cells and diagnosis of lymphomas and leukemia.

 

5. What are the normal functions of NK cells?

NK cells are cytotoxic cells. They are known for acting upon and destroying virus-infected cells, detect and control early signs of any cancer and limit cancer spread and tissue damage by killing tumour cells. As NK cells are present in the placenta, they also play an important role in maintaining pregnancy.

 

6. What is the relation between viral infections and NK cells?

NK cells influence the course of chronic viral infections like EBV, Hepatitis C virus, HTLV-1 and HIV – AIDS. Long-standing infection with these viruses harms NK cells leading to the development of structurally and functionally abnormal NK cells. EBV infects non-neoplastic NK cells, causing them to proliferate and can subsequently cause extranodal lymphomas, especially in pediatric and young adult age groups. This is collectively referred to as systemic EBV infection plus T-cell or NK-cell LPDs of childhood. A unique feature of EBV associated LPDs is the presence of severe inflammatory and necrotic skin reactions. HTLV-1 is implicated in adult T cell leukemia/lymphoma.

 

7. NK cells express which CD markers?

NK cells have an incomplete T-cell receptor complex. They are positive and express cytoplasmic CD3 (not surface), CD2, CD7, CD11b, CD11c, CD16, CD56, variably CD57, cytotoxic proteins and sometimes CD8.

 

8. What is the panel of testing for T cell LPDs?

When T cell LPDs are suspected, you need testing for CD2, CD3, CD5, CD7, CD4 (T helper cells), CD8 (T suppressor cells), HLA-DR, CD16, CD56, CD57, CD26 and CD25. Cytoplasmic CD3 expression is the gold standard flowcytometry marker of T-cell lineage.

 

9. Can I directly ask for this testing if I have any symptoms?

No. It is always the treating physician’s call. After correlating with other lab parameters, symptoms and clinical findings, the decision of doing NK cell testing is taken.
