About Test:

T-lymphocyte antigen, CD4 is a biomarker for normal T-helper cells and plays an important role in T-cell activation. It maintains peripheral tolerance and prevents autoimmunity. T-cells co-expressing both CD4 and CD8 are undergoing thymic maturation or migration and are present at very low levels in normal blood. Excess levels of CD4 positive cells may arise due to a congenital or acquired deficiency of CD8 positive cells and vice versa. This test is about counting the percentage and the absolute number of CD4 cells and it helps in monitoring the body’s T cell status. It gives us a broad overview of an individual’s immune system. CD4 cells are one of the important targets for the HIV-Human immunodeficiency virus. As HIV infection progresses CD4 cell count decreases and susceptibility to getting various opportunistic infections and complications increases.

This test is done by Flowcytometry analysis. In this test, first, the lymphocytes are separated from other white blood cells using CD45 gating. Then T lymphocytes expressing CD3 are separated out and subsequently absolute value and percentage of CD4 (Helper T) cells in a given sample are reported. The biological reference range for CD4% is 27-51 % and the absolute count is 448-1611 Cells/µL. It is advisable to consider both CD4 percentage and absolute counts in cases of discordant results. 

All reported values should be correlated with clinical features, various immunological parameters and relevant laboratory results. Before doing any treatment modifications in patients owing to an unexpected result, always repeat CD4 testing. There is profound CD4 depletion in HIV infection and it is always advisable to calculate absolute values of CD4 and CD8. Depending upon the CD4 values, a clinician will decide the treatment strategy to improve their status. CD4 values <400 Cells/µL increases the chances of getting opportunistic infections. In the case of LPDs, High CD4 counts suggest clonality and favour the diagnosis of that specific lymphoma. If values are lower or within range, it does not rule out the possibility of any lymphoma and needs further workup inclusive of a whole panel of CD markers for B or T lymphocytes depending upon the clinician’s suspicion.

Medical condition:

Indications for doing this test are as follows:

i. HIV infection: Serial monitoring is done in HIV patients to determine the stage, severity and outcome of the infection. It tells us about the patient’s immune system, disease progression, predict for future complications and effectiveness of antiretroviral treatment. It is essential to obtain baseline CD4 counts before starting HIV treatment.

ii. Classification and diagnosis of T cell lymphoproliferative disorders (LPDs) like Sezary syndrome/cutaneous T-cell lymphoma, T-cell prolymphocytic leukemia, acute T-cell leukemia/lymphoma, anaplastic large cell lymphoma, angioimmunoblastic T-cell lymphoma and indolent small intestinal T-cell lymphoma. Rarely it may also be seen in T-cell large granular lymphocyte leukemia and acute myeloid leukemia (M4, M5).

iii. T-cell immune monitoring in patients of organ transplantation, autoimmunity and other immune-related disorders who have received immunosuppressive and immunomodulatory therapies.

iv. To assess T-cell immune reconstitution after hematopoietic stem cell transplantation.

v. Screening, diagnosis and follow up of congenital disorders like DiGeorge syndrome, other primary cellular immunodeficiencies and severe combined immunodeficiency disorder.

vi. Seeing baseline counts in immunodeficient patients before starting antimicrobial therapy.

vii. Early screening to look for quantitative T-cell anomalies in infection or malignancy patients.

1. What are the factors affecting CD4 counts?

Factors affecting CD4 counts include circadian rhythm (day and night cycle), time of collection, age, sex, seasonal variation, menstrual cycle, pregnancy, ethnicity, any ongoing stress, recent surgery, the effect of drugs like chemotherapy, corticosteroids, immunosuppressive or anti-rejection medications, presence of anti lymphocyte antibody, splenectomy, smoking, infections, delay in analysis, sequential use of different assay and sample storage.  

 

2. For disease monitoring, is a single CD4 count sufficient?

CD4 counts in the blood may change up to 30% without any changes in clinical status due to biological and method variation. So it is advisable to do serial monitoring and observe trends over some time rather than a change in a single specific test result. In the case of HIV patients, CD4 counts every 3-6 months is advisable. It recommended to do follow up testing at the same time of the day to avoid the effects of diurnal variation.

 

3. Which type of LPD is more common?

Lymphocytes are of two types- B lymphocytes and T lymphocytes. In peripheral blood, 80-90% is T-cells. B lymphocytes constitute only a minority (10-20%) in circulating blood, but B-cell LPDs are more common as compared to their T-cell counterparts.

 

4. What is lymphoma?

Lymphomas are cancer that arises in lymphocytes. Lymphocytes are WBCs present in lymph nodes, thymus, spleen, bone marrow and specific lymphoid pockets all over the body. They are immune system cells that have varied functions right from fighting and preventing infections to the healing of infections and in autoimmunity. Lymphomas are of 2 types, viz. Hodgkin and Non-Hodgkin. In lymphomas, lymphocytes change, grow in an uncontrolled and uninhibited fashion, each category affects a different type of lymphoid cell and variable responses to therapy.

 

5. What is leukemia?

Leukemias are the cancer of blood-forming cells inside the bone marrow. Blood cells are constituents of blood which includes red blood cells, white blood cells and platelets. Their precursors are present in the marrow in an immature form which undergoes maturation to get released into the blood.

 

6. What is flow cytometry?

Flow cytometry is a highly specialized laboratory test done on a cytometer with the help of laser beam and fluorescent dyes or tagged substances (antibodies) to identify the types of cells in the blood, bone marrow or lymph node samples. It can also identify particular components within the cell and count the specific number of cells. Its most common use in medicine is immunophenotyping and diagnosis of leukemia and lymphomas. 

 

7. What does CD stand for?

CD stands for cluster of differentiation which is also known as cluster of designation or classification determinant. The pattern of surface CD antigen expression indicates the degree of maturity. It is a protocol utilized to identify and investigate molecules present on cell surfaces. This subsequently helps in providing targets for immunophenotyping of cells and diagnosis of lymphomas and leukemia.

 

8. What is the panel of testing for T cell LPDs?

When T cell LPDs are suspected, you need testing for CD2, CD3, CD5, CD7, CD4 (T helper cells), CD8 (T suppressor cells), HLA-DR, CD16, CD56, CD57, CD26 and CD25. Cytoplasmic CD3 expression is the gold standard flowcytometry marker of T-cell lineage. 

 

9. Can I directly ask for this testing if I have any symptoms? 

It is always the treating physician’s call. After correlating with other lab parameters, symptoms and clinical findings, the decision of doing CD5 testing is taken based upon suspicion of LPDs.
